Change support ticket form inputs based on ticket type.

Use case: Customer request slack channel. Our first reply to customer is to ask for an administrator email in their organization. It would help the customer if we could collect their admin email when they are first inputting the request for the slack channel. Generically, this should work for any ticket type - display inputs relative to that type of request.
